| Assertive Community Treatment Team |
|
The Assertive Community Treatment (ACT) Team responds to those mentally ill clients who have demonstrated a high need for
services which are provided as intensively as indicated.
Intensive Case Managers working on the ACT, work with a smaller caseload with the goal of
preventing hospitalization and sustaining the client in the community.
The ACT team corrdinates clients' treatment with other providers of physical health, social services, vocational training, residential services and economic supports. |
